I consent to their participation in the Gymnastics program as well as to the following basic rules:
The coaches are in charge of the floor area.
Parents and siblings are not allowed on the floor area.
Any emergency situations – accidents, injuries, fire, the senior (supervising) coach is in charge and all participants and parents will follow their instructions.
Photography is restricted to your own child only.

I understand, approve of the above application and in doing so agree that The Hub Gymnastics Club Inc. and its officers, leaders, staff and agents shall be released from and shall not incur any responsibility or liability whatsoever for any accident or injury to the applicant or for any damage to or loss of property of the applicant. I further authorize you to obtain medical/ambulance assistance in the case of accident or emergency involving the applicant and I agree to bear the costs thereby incurred.
