By making a booking I consent to the attendee/s participation in the Gymnastics program as well as to the following basic rules and agreement:

1) The coaches are in charge of the floor area.
2) Any emergency situations – accidents, injuries, fire, the senior (supervising) coach is in charge and all participants and parents will follow their instructions.
3) Photography is restricted to your own child only.


I understand, approve and in doing so agree that The Hub Gymnastics Club Inc. and its officers, leaders, staff and agents shall be released from and shall not incur any responsibility or liability whatsoever for any accident or injury to the applicant or for any damage to or loss of property of the applicant. I further authorise you to obtain medical/ambulance assistance in the case of accident or emergency involving the applicant and I agree to bear the costs thereby incurred.

I have supplied relevant medical details in my booking.

The Club requests you DO NOT visit if you have:
o Fever or symptoms of respiratory infection (cough, sore throat, shortness of breath).
o Returned from overseas/interstate in the last 14 days.
o Been in close contact with a confirmed case of COVID-19 (coronavirus) in the last 14
days.

If any participant or venue attendee attends with any respiratory symptoms, they will be denied entry/ separated from training. Participant parents/guardians will be contacted immediately and asked to collect their child.
