You have given permission for your child to register with Kickstart Soccer for the sole purpose of participating in sporting sessions with Kickstart Soccer ("Kickstart"). You have agreed to indemnify Kickstart and its employees, volunteers, its officials and servants from and against any claims or demands whatsoever in respect of any illness or injury which might befall your child during his/her participation in activities conducted by Kickstart. Such activities include but are not limited to soccer training and matches, including the transport of your child to and from such activities. Furthermore, you have authorized Kickstart in the event of illness or injury to your child, if you are unable to be contacted, to obtain medical, hospital or similar assistance deemed necessary by Kickstart, and you will accept full responsibility for all costs associated with such assistance. You have agreed that Kickstart does not reimburse persons who incur damage to their property during the course of activities organized by Kickstart such as training, sporting camps, social events, or matches. By registering your child with Kickstart, you hereby give permission for Kickstart to use the name and images of your child in the end-of-year report, yearbook, results, team photos, press releases, club newsletters, on official websites, or any other use deemed suitable by Kickstart. You have also agreed that there is no obligation for Kickstart to refund any registration monies if the child is unable to attend or the event is cancelled due to weather. If your child requires special needs support or has behavioral or anger management issues, a parent or carer must be present to support the child during the session.
