As the Youth Centre Member’s Parent or Guardian, I agree to the following Conditions.

☐ I acknowledge that while the Shire of Boddington takes reasonable steps to minimise the risk of personal injury, accidents can occur, and physical activities inherently carry some level of risk. I understand the potential for injury during this excursion and accept that my child participates in these activities at their own risk.

☐ In the event of a medical emergency, I authorise the Shire of Boddington staff to provide immediate assistance and seek appropriate medical care for my child, including transport by ambulance or a Shire of Boddington vehicle. I agree to cover any costs associated with medical treatment or transport.

☐ I agree to provide all necessary medication for my child. I acknowledge that Shire of Boddington staff do not administer medication but can supervise my child while they take their medication as needed.

☐ I agree to be contactable on the phone number provided, for the entire duration of the excursion. I understand that it is my responsibility to remain reachable in case of emergencies or important updates regarding the excursion.

☐ I acknowledge that it is my responsibility to promptly inform the excursion staff of any changes to the contact information or other details provided on this form.

☐ I acknowledge that the Shire of Boddington will not be held responsible for any loss, damage, or theft of personal property during the excursion. Furthermore, I understand that if my child causes damage to property, including but not limited to transport vehicles, event venues, or equipment, I, as the parent/guardian, will be held responsible for covering the cost of repair or replacement as determined by the affected parties.

☐ If my child behaves irresponsibly or fails to follow safety rules, I agree to collect them or cover costs for their return. I understand their behaviour may result in disciplinary action and exclusion from future Youth Centre activities or events.

☐ I give consent for photos/videos of my child participating in this excursion to be used for youth centre promotional purposes (e.g., newsletters, social media, website).
