Medical Consent: I, being the parent/guardian of the above child(ren) understand that whilst every precaution will be taken to ensure the good welfare and protection of my child(ren), Rockdale Uniting Church (RUC), it’s staff and volunteers acting on behalf are hereby released from any and all liability in the event of any accident or misfortune, damage or loss that may occur to the child(ren) or their property. In case of emergency, I give permission for first aid to be given to ensure proper treatment to my child(ren).

Photography and Video Consent: I, being the parent/guardian of the above child(ren) give permission for them to be captured in both video and photographs of RUC Kidsfest. RUC Kidsfest reserves the right to use the material for promotional purposes.
