In completing this booking I acknowledge that:

All players and spectators must adhere to the Lakeside Recreation Centre's Code of Conduct and at all times act respectfully towards all Staff, Referees, Opponents, Teammates and Spectators.

Lakeside Recreation Centre has a zero tolerance policy in regards to audible swearing and obscene or abusive language or behaviour.

Lakeside Recreation Centre reserves the right to take photographs and videos during the course of the competition for use in promotional material. By ticking below I am giving consent that all photos and videos taken of myself and all players participating in Lakeside's 3x3 competition can be used on promotional material as well as promotions on the Lakeside website and social media accounts.

Teams who choose a team name which is deemed inappropriate will be asked to choose a new team name prior to acceptance into the competition.

Team Nomination fees may only be refunded at the discretion of Lakeside Management. Teams will not receive refunds for change of mind or if they become unavailable. Please let us know if you need to substitute a player if a player is no longer able to play due to illness or injury.

I have read the rules regarding age limits (shown below) and that if I register my team into the wrong division (or age), my registration will be void with no refund.
Under 12s - All players should be turning 11 or younger in 2026 (Born in 2015 or later). If born in 2014 or earlier they cannot play in U12.
Under 14s - All players should be turning 13 or younger in 2026 (Born in 2013 or later). If born in 2012 or earlier they cannot play in U14.
Under 16s - All players should be turning 15 or younger in 2026 (Born in 2011 or later). If born in 2010 or earlier they cannot play in U16.
Under 18s - All players should be turning 17 or younger in 2026 (Born in 2009 or later). If born in 2008 or earlier they cannot play in U18.
Under 23s - Players should be turning 22 or younger in 2026 (Born in 2004 or later). If born in 2003 or earlier they cannot play in U23.
