Acknowledgement of Risk
I understand that participation in Australian Rules Football (AFL) training with Level Upp Coaching involves physical activity and that injuries may occur, including but not limited to strains, sprains, collisions, fractures, or other bodily harm.

Voluntary Participation
I confirm that my child is participating voluntarily and that they are physically fit and able to safely take part in AFL training sessions.

Assumption of Responsibility
I accept full responsibility for any injury, loss, or damage that may occur to my child or their property during participation in AFL training sessions conducted by Level Up Coaching.

Release of Liability
To the fullest extent permitted by law, I release and hold harmless Level Up Coaching, its coaches, organisers, volunteers, and any training venues from all claims, demands, or liabilities arising out of my child’s participation in AFL training.

Medical Consent
In the event of an injury or emergency, I authorise Level Up Coaching staff to seek or provide appropriate medical assistance for my child, and I agree to be responsible for any related costs.

Agreement
By ticking this box, I confirm that I am the parent or legal guardian of the participant, that I have read and understood this waiver, and that I agree to all the terms and conditions above.
