This is an important document which affects you/your child’s legal rights and obligations. Read it carefully and sign.

Understand that SYDNEY CIRCUS COMPANY at 1/42 William Street, BOTANY NSW 2019 runs classes, workshops, performances, parties, and training that is potentially dangerous to the named participant. By participating in them, I/my child am/is exposed to certain risks.

I ACKNOWLEDGE and UNDERSTAND that whilst participating in such activities I/my child:
May be injured physically or mentally, or even fatally.
Personal property may be lost or damaged.
Other persons participating in such activity may cause me/my child injury or may damage my property.
I may cause injury to other persons or damage their property.
May be injured or suffer fatality or damage to my property because of my/my child’s own negligence or breach of conduct with SYDNEY CIRCUS COMPANY.
Assume the risk of, and responsibility for any injury, fatality or property damage resulting from my/my child’s participation in the activity.
Acknowledge I am physically and mentally healthy, I assume all responsibility for my personal fitness in participating in all activities and services.
Must advise a staff member before class if I/my child am/is injured, have a pre-existing condition which may limit my/my child’s ability to participate in any way.
I consent to the use of any photos of me/my child, taken while participating in activities provided by SYDNEY CIRCUS COMPANY for advertising, promotion or posterity.

Any participants who engage in the above activities without the guidance of a SYDNEY CIRCUS COMPANY instructor, trainer, teacher is doing so at their own risk. I/my child acknowledge that any activities may be averse to SYDNEY DANCE COMPANY’S proper safety instruction, protocol, conduct and advice.

I agree to release and indemnify SYDNEY CIRCUS COMPANY, its employees and directors from any injury, loss, damage or fatality that may be caused by my decision to participate in these activities.

In consideration to permitting me/my child to participate, use the facilities and equipment at SYDNEY CIRCUS SCHOOL, I agree to release and indemnify SYDNEY CIRCUS COMPANY as follows:
Participate in the activities at my/my child’s sole risk and responsibility.
Release, indemnify SYDNEY CIRCUS COMPANY its directors, employees and agents, from and against all and any actions or claims which may be made by me or on my behalf, or by other parties for or in respect of or arising out of any injury, loss, damage or fatality caused to me/my child, or my/my child’s property by my/my child’s own negligence, breach of contract or in any way whatsoever.
I also AGREE that in the event I/my child am/is injured, or my/my child’s property is damaged, I will bring no claim, legal or otherwise, against SYDNEY CIRCUS COMPANY in respect of that injury or damage.

Before signing this document, I have read and understood it and know that it affects my Legal Rights.
