I understand the refunds are not available for non-attendance on the day. Refunds may be issued at the discretion of Prelude Dance Centre if they are contacted 24 hours before the event.

Prelude Dance Centre reserves the right to change Event day at any time. If an alternative cannot be found an event may need to be cancelled.

Photos of your child may be used in Prelude Dance Centre advertising and promotional materials UNLESS you specify otherwise.

I agree to accept full responsibility for any injury sustained by myself or my child while attending this event, for any injury to my child or children in the waiting areas, and for any loss or damage of property.

I agree that in case of an emergency Prelude Dance Centre has my permission to seek medical aid and I agree to pay any expenses incurred.

I understand and accept that due to the nature of this event that some physical contact may be necessary by a teacher to demonstrate activities or if there is a need to administer first aid.
