1. Participation:
I consent to my child’s participation in this excursion. I have been informed by the school of the arrangements made for the conduct of this excursion. I understand that I have given permission for my child / children to attend Turning Pointe Dance Studios, Innisfail Troupes Rehearsal on Sunday 18 April 2021. I agree to the condition that whilst all care will be taken when supervising students at all times, Turning Pointe Dance Studios, and its Director or staff, will not be liable or responsible for any accidents or injuries.
2. Medical:
In the event of accident or illness when it is impracticable or impossible to communicate with me, I understand the teacher in charge will arrange such medical treatment as may be deemed necessary.
3. Transportation:
I understand that my child will be transported to and from Innisfail via coach with Cairns Bus Charters.
4. Expenses:
• I agree to reimburse the school for any wanton damage caused by my child.
• I agree to reimburse the school for any hospital, medical or ambulance expenses incurred by the school on behalf of my child.
