I have read the Flourish Programs Information Sheet and I also understand that:
• I will lose my place in the program if I do not adhere to the attendance requirements.
• I need to send an apology if I’m going to miss a session. (Ahead of time if possible)
• Still Images, video and audio clips may be taken and used for promotional and reviewing purposes.
• Flourish Programs adhere to the “ChildSafe” policy guidelines and will complete any mandatory reporting to keep all children safe.
We take every effort to keep your child safe
Flourish Programs agrees to take the best possible care of your young person during their time with us. The program will have an adequate number of trained leaders to supervise your young person at all times during the program. We take our Duty of Care for your young person seriously and will always put their safety ahead of any other priority.
Authorisation
I authorize the leader in charge of Flourish/Junior Leadership Programs to arrange for my young person to receive such first aid, medical or surgical treatment as the leader may deem necessary at any time during Flourish Programs activities. I further authorize the use of Ambulance and/or anaesthetic by a qualified medical practitioner if in his/her judgment it is necessary. I accept responsibility for payment of all expenses associated with such treatment.
I recognise that being part of a community involves mutual care and consideration and therefore agree that any behaviour which the program deems as unacceptable may result in my young person being sent home and/or being temporarily prohibited from attending any Flourish Programs.
I understand and agree that the personal & emergency information provided will be used only for the purposes of contacting me/my young person or to care for my young person in the case of an emergency.
I understand and agree that photographs & video footage of students for promotional & archival purposes may be taken by the program during Flourish events. (Promotional platforms include our website, our social media pages and hard copy collateral. If this concerns you or you would prefer your young person not to be included in this material please contact the program director Emily Fairweather on 0430 300 181 or info@flourishprogram.org.)
I understand that Flourish Programs are being hosted by SandyLife Baptist Church which is a recognised Christian church endeavouring to serve the local community by offering services, and programs that are relevant to all ages in the community. All Flourish Program participants need to respect the buildings, grounds and volunteers from the Church.
I understand that all Flourish Programs are an Alcohol, Smoke & Drug Free program and I will encourage my young person to cooperate with the Flourish leaders and to follow the Flourish values below.

Flourish Participant Agreement
I understand that the Flourish Programs are an Alcohol, Smoke & Drug Free program and I agree to cooperate with all Flourish leaders and other young people and to follow the Flourish Values which are:

5. I will engage in positive behaviour during the Flourish Program.
