By completing this booking attendees agree to the Medical Condition Disclosure and Event Waiver detailed under Terms and Conditions of this booking. As part of registration, each attendee will be required to complete a mandatory Medical Disclosure question.
MEDICAL CONDITION DISCLOSURE AND EVENT WAIVER
AHISA Limited (ACN 006 107 124) (AHISA) is conducting the AHISA (V) BRANCH MEETING AND LUNCH held at RACV CITY CLUB MELBOURNE on TUESDAY 26 MAY 2026.
As part of that Conference, AHISA may offer or arrange certain recreational events and activities (Events) for the participation of Members, their spouses and others which may pose inherent risks to persons/property and/or contain an element of physical activity.
By making a booking for this event, the attendee/s acknowledge that Events are optional, you are fit and able to participate in events of your choice, and that you have read and agreed to the following:
1. Event Provider: AHISA will typically arrange third parties (Event Providers) to supply Events. I understand that where an Event is supplied by an Event Provider, the Event Provider may have its own terms and conditions for participation in the Event. I am fully responsible for abiding by those terms and conditions.
2. Voluntary: I understand that my participation in Events is voluntary.
3. Participation at own risk: I am aware that Events may involve physical activity that may be inherently dangerous, and I freely and voluntarily participate at my own risk. The risks I may be exposed to during Events include, but are not limited to, overexertion, equipment failure, dehydration, accidents and inclement weather. Such risks can occur and may result in personal injury, death or property damage. I accept and assume responsibility for these risks. I understand that if I have any concerns about my fitness or ability to participate in an Event, or my safety, then I should choose not to participate in the Event.
4. Exclusion of Event Provider liability: The Australian Consumer Law guarantees may not apply to certain Events that are recreational services where a relevant prescribed statutory warning is given to participants or legislation allows the Event Provider to exclude liability. I understand that the Event Provider may ask me to sign a waiver to exclude, restrict or modify their liability for any personal injury suffered by me.
5. Extension of signed waiver: To the extent permitted by law, I agree to the benefits of any signed waiver in favour of an Event Provider being extended to AHISA. The terms of any waiver are otherwise not incorporated in this document, and do not give rise to any obligation on the part of, or right against, AHISA.
6. Release and indemnity: To the extent permitted by law:
(a) I release and forever discharge AHISA and its officers, employees and agents from all Claims that I may have now, or may have in the future but for this release, arising from or in connection with my participation in the Events; and,
(b) I indemnify and will keep indemnified AHISA and its officers, employees and agents in respect of any Claims by any person (including another participant in Events) arising from or in connection with my participation in the Events.
In this clause 6, “Claims” include any action, suit, proceeding, claim, demand, damage, penalty, cost or expense however arising, including (but not limited to) negligence.
7. Declaration of fitness to participate: I declare that I am, and will stay, medically and physically fit and able to participate in Events. I also declare that I am not, and will not, be a danger to myself or others. I have disclosed in this document any medical or physical conditions to AHISA that may be relevant to my participation in an Event, and I will immediately notify AHISA and the relevant Event Provider of any change to my fitness or ability to participate. I acknowledge that AHISA has facilitated my voluntary participation in the Event and is relying on this document as evidence of my fitness and ability to participate.
8. Consent to medical treatment: I consent to receiving medical treatment that AHISA considers reasonably necessary or desirable during or after Events. I will reimburse AHISA for the cost of arranging such medical treatment.
9. Weather and unexpected conditions: I acknowledge that Events may be affected by weather and unexpected conditions beyond AHISA’s or an Event Provider’s control. In the case of inclement weather or unexpected conditions, AHISA or an Event Provider may reserve the right to suspend, change or cancel Events at their discretion. AHISA is not liable as a result of such suspension, change, cancellation or non-delivery of Events.
10. Security of personal possessions: I am responsible for the security of my personal possessions (including valuable items) at all times during Events. AHISA will not be liable for any loss or theft of my personal possessions.
11. Privacy: I acknowledge that my personal information will need to be used by AHISA and disclosed to the Event Provider to facilitate the conduct of Events.
