Indemnity Form and Terms & Conditions

Important: In the event of accident or illness, I authorise the staff member(s) in charge to consent, where it is impracticable to communicate with me, to my receiving such medical or surgical treatment as may be deemed necessary, transportation and/or hospitalisation and agree to pay all charges incurred.
I understand that in the event of property damage I am not covered under the auspices of the institute insurance and neither I nor a person representing me will make a claim against the institute or any individual on the staff of the institute.
1. The full payment must be paid before a place on the activity/trip is registered.

2. If the organiser cancels the activity/trip, all fees will be refunded directly to your credit card via Eventbrite and the participant will be notified by email or telephone. If payment is made in cash, in person, you will be contacted by a SLAM staff member.

3. If an activity/trip becomes fully booked, the student will then be put on a waiting list. Listed students will be notified if a position on the activity/trip becomes available by SLAM staff member.

4. Cancellations and Refunds - If you withdraw from the trip you must inform SLAM in writing and attach all payment receipts. Melbourne Polytechnic will keep cancellation fees as follows:
- 15 days or more before departure 50% of total paid
- 14 days before departure NO REFUND
NB: A full refund will be issued if you find a replacement student for the trip. If minimum numbers for trips or activities are not met they may be cancelled and a full refund will be issued.

5. Trips/activities are open to all enrolled students unless otherwise stated. If the trip is not fully booked at the cut-off date, students may be able to bring a friend or spouse who is not a student if they are sixteen years of age or over.

6. Participants agree to abide by directions given by Melbourne Polytechnic staff and all associated venue or activity staff. Failure to maintain appropriate behaviour standards may result in exclusion from the remainder of the activity and further disciplinary action according to the Student Discipline Policy. Disruptive participants may be sent back to the institute at their own cost.

7. Students with special needs should discuss with staff the suitability of the activity and their particular requirements prior to booking. Any special requirements (physical, medical) must then be identified with SLAM via email slam@melbournepolytechnic.edu.au
