DISCLAIMER

Participating in dance classes may involve risk of personal injury. Flashback Dance is not a licensed medical care provider and represents that it has no expertise in diagnosing, examining, or treating medical conditions of any kind, or in determining the effect of any specific exercise on a medical condition.

Flashback Dance reserves the right to amend or cancel classes without prior notice due to unforeseen circumstances.

LIABILITY WAIVER

By registering to attend any classes with Flashback Dance, you agree to the following:
1. I recognise that participating in Flashback Dance classes requires physical exertion that may cause injury. I knowingly, voluntarily and expressly waive any claim I may have against the facilitators individually or otherwise, of Flashback Dance for damages, and injury that I may sustain as a result of participating in classes.
2. I participate knowing that I am physically fit and have no medical conditions that would prevent my participation in the classes.
3. I understand that it is my responsibility to advise the facilitator of any injuries, or medical considerations so the class content can include suitable modifications where required.
6. I forever release, waive, discharge and covenant not to sue Flashback Dance for injury or death caused by their negligence or other acts.
7. I will not hold responsible/sue Flashback Dance facilitators for any injuries suffered by me caused whole or in part by my failure to follow the instructions of the facilitators or by any physical impairment of mine not fully disclosed to you in writing.
8. I understand that it is my responsibility to abide by the rules of the safe space plan* whereby practices will support a covid-safe premise.

MEDIA RELEASE

I consent and authorise Flashback Dance to use my image for photos and video footage for any publications, print, web design and social media. I authorise Flashback Dance to copy, edit, enhance, crop or otherwise alter any photo for use in publications. I also waive any rights for approval or inspection of any photos. I understand and agree that all photos are the property of Flashback Dance. I acknowledge that I am not entitled to any compensation or royalties with respect to the use of the photos.

- If you are feeling unwell at all, please stay home. Please cancel your booking prior to class commencing so that your spot can go to another participant and your pass will not be deducted.
- Please note that Flashback Dance accepts both vaccinated and unvaccinated participants. This is in accordance with the QLD Government COVID Safe Checklist—Unvaccinated staff and patrons permitted.
