By purchasing a ticket I give permission for my child/children to attend the Belmont climbing outing, and for the Leaders in charge on the night to seek medical assistance for my child/children should the need arise.

I will advise Friday Night Youth Leaders of any dietary or medical requirements that are relevant for the night.

I agree to drop by child/children at Parkerville Baptist Church at 5.40pm in time for the coach to depart at 6pm and pick them up again at 9.45pm from the Church. I understand that if they miss the coach I will need to arrange my own transport to Adrenaline Vault Belmont.

I understand that if my child does not follow the Parky Youth rules and guidelines on the night or in case of emergency the emergency contact as stated on this form will be contacted to pick them up from Adrenaline Vault Belmont.
