Participation Medical Waiver
Acknowledgement of risk
I acknowledge that I am a willing participant in the “Battle of the Saints” football game and understand I play at my own risk with the potential for injury. It is highly recommended to wear both a mouth guard and appropriate football boots throughout the game.
If there is any health concern, it is recommended you consult you GP before participation.
Release of liability
Participants release the event organizers, primary schools and venue from liability for any injury or medical event that may occur during the event.
Medical Authorization
I understand that there will be first aid officers / volunteer health professionals such as nurses /physiotherapists/osteopaths etc who will act in good faith and administer first aid if needed on the day. I acknowledge that any treatment from these professionals is provided for free on the day ,and I absolve them from any liability.
Code of conduct
I understand that this is a social game of football and should be played in a friendly manner. Any reckless or dangerous behavior or play that could result in injury or harm to others is not tolerated and may result in being carded off the ground and also possibly barred from any future games.
Ongoing Liability or Injury
In the event of any medical episode or injury occurring, I understand that any treatment or surgery required will be at my own personal expense.
I understand that there is no liability placed upon the venue / local council / my child’s school or the organizers of this event with regards to any injury or mishap relating to my participation in battle of the Saints Football game.
It is highly recommended participants also have adequate private health cover.
