Information about your vaccination will be collected by the Victorian Department of Health. The Department may use and disclose this information to create a record of your vaccination, retrieve your vaccination history from and update the Australian Immunisation Register, facilitate your next vaccination, notify SAEFVIC and AusVaxSafety of any adverse reactions, provide you with a survey, and monitor and manage vaccinations across Victoria.

The consenter agrees to the vaccinee receiving a full course of the vaccination(s), and to the Department collecting their health information, using and disclosing that information for the purposes set out in the privacy collection statement.
