I understand that some of the exercises and activities involved in workshops may involve risk. I understand that in any case of a serious accident, if my emergency contact is unavailable; I will be transported to a hospital by ambulance.
I hereby indemnify and hold harmless, Swag House / Flava Dance Studios, its staff and teachers (Josh Cesan & Hamlin Croft ) in respect of any injury I may occur because of participating in this class/s. I am aware that any cost involved for this and medical treatment will be payable by me.
I understand that images or videos taken may be used for promotional purposes.

IN THE EVENT THAT THE WORKSHOPS ARE CANCELLED, ALL DANCERS WILL BE NOTIFIED & REFUNDED.
