By purchasing a ticket you give permission for your child named on the ticket to participate in the Good Shepherd end of year Disco, held at the Good Shepherd Performing Arts building.
In the event of any illness or accident where I am unable to be contacted, I authorise a parent in charge of the disco to consent to my child receiving such medical assistance as is recommended by a medical practitioner. I also authorise the parent in charge of the activity to administer first aid as considered to be reasonably necessary.
I accept all risks involved and accept responsibility for payment of all expenses incurred in relation to such treatment and any emergency transportation required.
