Melbourne RG Academy – Participation Waiver, Consent and Media Release

By signing below, you acknowledge and agree to be bound by the terms, conditions, policies and procedures available on the Melbourne RG Academy website.

You understand and agree that participation in rhythmic gymnastics, fitness, dance, holiday programs, competitions, performances and related activities involves physical activity and inherently carries a risk of injury. You should instruct the gymnast to follow all safety rules and directions provided by Melbourne RG Academy coaches and staff at all times.

You hereby provide your consent for yourself and/or your child(ren) to participate in all Melbourne RG Academy activities and accept all risks associated with that participation. To the fullest extent permitted by law, you acknowledge that Melbourne RG Academy, its directors, employees, coaches, volunteers and representatives will not be liable for any injury, loss or damage arising from participation in club activities, except where such liability cannot be excluded by law.

All athletes participating in Melbourne RG Academy programs under the instruction of Melbourne RG Academy coaches are covered by the Gymnastics Australia National Risk Protection Program, subject to the terms and conditions of that program. Photography and Video Consent

Melbourne RG Academy requests permission to photograph or video athletes during classes, training sessions, holiday programs, competitions, performances and other club activities for the purpose of promoting the club and its programs.

The athlete's full name, class day or class time will not be published alongside their image. Captions accompanying images will be general in nature, for example: "Melbourne RG Academy athletes practising apparatus skills during training."

By signing below, you give permission for yourself and/or your child(ren) to be photographed or filmed for marketing and promotional purposes. You understand that these photographs and videos may be used by Melbourne RG Academy in:

Club websites
Social media platforms
Newsletters
Posters and flyers
Digital and print advertising
Competition programs and event publications
Other promotional and marketing materials

You understand that you may withdraw this permission at any time by contacting Melbourne RG Academy in writing. Withdrawal of consent will apply to future use of images and videos and will not affect materials already published.

You may request copies of photographs featuring yourself or your child by contacting Melbourne RG Academy.

Acknowledgement

I confirm that I have read and understood this waiver and consent form and agree to the Melbourne RG Academy policies and procedures as outlined on the club website.
