My agreement of these terms and conditions indicates my willingness to permit the applicant camper to participate fully in all activities associated with the FUSE Camp for 18th-20th September 2026, including, (but not necessarily limited to), those indicated in the brochure.

I assume full responsibility for the applicant camper’s health being such that the activities of the program will in no way aggravate any condition present. If in any doubt, I will seek to follow medical advice and inform the FUSE Camp of that advice. I will also notify the FUSE Camp of any significant change in the applicant camper’s health prior to the commencement of the program. I declare all information provided on the applicant's health to be true and accurate and that all relevant information has been provided.

I understand that while every precaution shall be taken to ensure the good welfare and protection of the applicant camper, the Anglican Diocese of Sydney, its council members, staff members, employees, or any other person acting on their behalf are hereby released from any and all liability in the event of any accident or misfortune that may occur to the applicant camper or damage or loss to their personal property.

In the case of a medical emergency, I hereby give permission to the doctor chosen by the Camp Director to secure proper treatment for and / or any hospitalisation, injection, anesthetic, or surgery for the applicant camper as named. I agree to pay all such doctors' and hospital fees incurred on behalf of the applicant. I understand that every effort will be made to contact me prior to instituting such procedures.

I understand that photos and video footage of activities may be taken and viewed within the youth ministry groups of Jannali Anglican Church and/or for future promotional purposes. I further understand that no such visual imagery recorded by FUSE CAMP 2026 leaders which singles out the applicant will be uploaded to the internet.
