I declare that my child is in a fit state of health to participate in this event at Knox Gymnastic Club and agree to accept sole responsibility of my Childs present condition. Prior to attending the special event, I hereby release Knox Gymnastic Club, its management, employees, agents and representatives from any responsibility for loss, damage, personal injury and liability of any kind which I or my child may sustain whilst entering or on leaving the premises. I further authorise you to obtain medical/ambulance assistance in the case of accident of emergency involving the applicant and I agree to bear any cost thereby incurred.

Privacy Statement - Knox Gymnastics Club uses this form to collect personal information for the purposes of program enrolment and statistical recording. The information may be shared with funding agencies, trainers and administrators. You will be able to access and amend or correct information on request.

Rights to Use Image - During the course of this activity, photographs and videos will be taken of the people participating in the program. Knox Gymnastics requests the right to use these photos in the promotion of this Special Event and the associated messages of participation in other Centre promotions.

I hereby grant permission to Knox Gymnastics Club Inc, the right to use my physical likeness without restriction in any promotion or promotional material created by or for Centre programs or events.
I accept that no fee or remuneration will be provided for my appearance in any Knox Gymnastics Program and/or related promotions and grants unlimited use of my image for this purpose only.
