1. I have current insurance through my club including Participation (Member to Member) & Sports Injury / Accident cover.

2. I understand I need to provide my own protective equipment and there will be no equipment available to borrow from the camp organisers.

3a. I consent to Kaizen Culture Ltd collecting media, that I may feature in, throughout this events and acknowledge that Kaizen Culture Ltd holds the rights to this media for the purpose of any legal use.

3b. I understand that I may revoke this authorisation at any time by notifying Kaizen Culture Ltd in writing. The revocation will not affect any actions taken before the receipt of this written notification. Media will be stored in a secure location and only authorised staff will have access to them. This will be kept as long as they are relevant, and after that time, destroyed or archived.
