MEDIA CONSENT FORM
By attending the formal consent is given to Thomas Hassall Anglican College to take or have taken by others, photographs, digital images and/or audio and/or video footage (the images) of the student attending and to store the images, make copies of the images and publish the images in any form, in whole or in part, and distribute them in any medium including, but not limited to, print media, the Internet, other multi-media uses or graphic representation or video.
Thomas Hassall Anglican College undertake not to use any images in a way that would cause embarrassment or misrepresent the intent of the student’s participation. I understand that neither I nor the student will be paid for giving this permission or any other payment in respect of use of the images.
VENUE RULES & CONDITIONS
For your safety and the safety of all guests, once you enter the venue you must stay inside the venue premises at all times.
Any guests who leave the venue premises will not be allowed re-entry under any circumstances.
Dockside adheres to the laws regarding responsible service of alcohol and respectfully has designated that all School Formals are STRICTLY NON-ALCOHOLIC for students. Persons will be removed by security if any unusual behaviour is observed that relates to the consuming of alcohol.
Dockside is a smoke free & gum free zone. These items will be confiscated on entry.
Balconies will be closed off if loud disruptive behaviour occurs, or objects are thrown over the balcony.
