Parent/Guardian Permission, Waiver and Acknowledgement of Risk
I, (full parent/ guardian name): hereby give permission for my child,(full child name): _ , to participate in the Grade 5 & 6 AFL-Style Football Game between Our Lady of the Nativity and St John Bosco, to be held at Aberfeldie Park West Oval on Saturday, 23 August 2025.
As the parent or legal guardian of the child, I acknowledge and agree to the following terms:
1. Assumption of Risk
I understand that participation in an AFL-style football game involves physical activity and carries inherent risks of injury or mishap. I voluntarily accept full responsibility for any injuries, mishaps, or health conditions that may arise from my child’s participation.
2. Medical Coverage
In the event that my child requires medical assistance or emergency medical services, including ambulance transport, I confirm that I have appropriate coverage and will be personally liable for any associated medical costs.
3. Liability Waiver
I release and hold harmless the event organisers, the affiliated schools (whether the event is sanctioned or not), volunteers, and associated personnel from any liability for injury, loss, mishap, or damage resulting from:
• My child’s own actions or inactions;
• The actions or inactions of other participants;
• Any accidents or incidents occurring during the event.
4. Responsibility for Conduct
I acknowledge that my child is expected to behave responsibly and respectfully, and I will ensure they understand the importance of taking reasonable care to avoid causing injury to themselves or others.
5. Fitness to Participate
I confirm that my child is physically fit and capable of participating in this event without limitation or assistance. I will not allow my child to participate if they are unwell, unfit, or under the influence of any substances that may impair their ability.
6. Compliance and Behavioural Conduct
I agree that my child will:
• Comply with the match rules;
• Demonstrate respectful and sportsmanlike behaviour towards teammates, opponents, match officials, volunteers, and organisers;
• Refrain from any disrespectful, aggressive, abusive, or inappropriate conduct.
• I understand that failure to adhere to these standards may result in removal from the game or future events.
