It is my choice to receive a Zenthai Shiatsu treatment.

I realise that the treatment is being given for the well-being of my body, emotions and mind.

I agree to communicate with the practitioner any time I feel like my well-being is being compromised.

I understand and am informed that in the practice of Zenthai Shiatsu there are some possible physical, emotional and mental side effects that may occur.

I do not expect the therapist to be able to anticipate and explain all risks and complications.

I rely on the therapist to exercise their best judgment during the course of the treatment, which they feel at the time, based upon the facts then known, is in my best interest.

I further understand that results are not guaranteed.

I understand that Zenthai Shiatsu therapists do not diagnose illness, disease, or any physical or mental disorders, nor do they prescribe medical treatment or pharmaceuticals.

I acknowledge that Zenthai Shiatsu is not a substitute for medical examination or diagnosis, and that it is recommended that I see a primary health care provider for that service.

I will state all medical conditions that I am aware of and will update my Zenthai Shiatsu therapist of any changes in my health status.

I acknowledge that the practitioner shall not be deemed responsible or liable for any injury, illness or mishap I sustain arising from or out of, or in any way directly or indirectly connected with the treatment given at the venue.
