The P&C will do it's best to provide a biscuit to each child and work around allergies. For complex allergies, the P&C may contact the parent to clarify ingredients. By filling out this form, you are giving permission for your child to receive a biscuit. If a form (electronic or paper) is not received, the P&C regrets it cannot provide your child with a biscuit due to duty of care relating to personal allergies.
