I agree to allow the leaders of Christ Church St Ives to accompany my child (in a group no smaller than 4) around St Ives by foot for this organised scavenger hunt.

In the event of accident or illness, I authorise the Youth and Families Minister to consent, where it is impracticable to communicate with me, to my son/daughter receiving any x-ray examination, anaesthetic, medical, surgical or hospital treatment as may be deemed necessary by a licensed physician and/or surgeon. I also authorise them to engage such treatment and agree to pay the appropriate fees for such service and treatment.

I give permission for the use of any photos or video footage of my child whilst at Year 6 Super Amazing Adventure Day in accordance with the Christ Church St Ives Social Contact Policy. For exceptions, please contact Jesse Baker via youth@christchurch.com.au
