By registering the child I agree to him/her participating in all supervised activities.

By attending Friday Fun Night there is an inherent risk within activities undertaken as part of the event. In the event of accident or illness, I authorise for the Youth and Families' Minister or Team Leader in charge to consent, where it is impracticable to communicate with the child's parent or guardian, to the child receiving any medical or surgical treatment as may be deemed necessary by a licensed physician and/or surgeon. I authorise the event leader to engage in such treatment and agree to pay the appropriate fees for such service and treatment. I agree to meet the expense of the child being returned home, either by the leaders accompanying him/ her and then rejoining the group or by collecting him/her personally. I understand that such an arrangement may be necessary due to illness, injury, or if, in the opinion of the Team Leader, non-cooperation or anti-social behaviour of any description, or inability to meet the rigours and requirements of the event activities by the child. The Team Leader will make every reasonable effort to communicate with you concerning any such behaviours or circumstances.

By registering the child on, Manly Waterworks, I agree to the use of any photos or video footage of the child in accordance with any permissions I have previously given with regard to the Social Contact Policy.
